At your first appointment you will need to provide the following completed form before any treatment can be given. If you cannot download the form please contact the office on 01527 892216 and we will fax or post a form to you.
Please note you will still be charged the full fee if we arrive at your stables and you do not have a competed form (meaning treatment cannot go ahead). This is to safeguard the well being and health of your horse.
